Answer 1: It's a tossup between Iijima Ai and Aoi
Sora.
Answer 2: Japan hasn't really known any sword masters.
Sure, they've mingled at cocktail parties, faced off
in company softball tournaments, etc., but Japan's
never really had a chance to intimately get to know
one. Acquaintences, you could say.
Answer 3, sans sarcasm: Asking this question is even
worse than asking "Who's the best quarterback to have
played in the NFL?" or "Who's the best NBA player of
all time?" or even "Who's the dumbest French soccer
player ever to head butt an Italian and get thrown out
of the World Cup Final?"
The answer that 90% of the world will give you is
Miyamoto Musashi, because that's who they've heard of.
I would also encourage you to do some research and
look up Tsukahara Bokuden and Kamiizumi Nobutsuna (or
Kamiizumi Hidetsuna, as he was also known), who
preceeded Musashi. The Yagyuu family also developed
some quite excellent swordsmen.
Who was best? Well, unlike sports we can't really
compare statistics--there are no passing yards or
points per game numbers that we can use. Read up about
them and decide for yourself.
